Printable blood glucose log sheet
Blood glucose log sheet, printable and app
Download a free blank blood glucose log sheet as PDF or CSV for handwritten or spreadsheet records. Glytrio also keeps the same glucose, food and daily-context fields in a private iPhone logbook, with factual PDF or CSV exports for a doctor visit.
Use one blood glucose log sheet row per reading
Use one row per reading. Record the date and time, the number exactly as shown by your source, and the unit you use: mg/dL or mmol/L. Do not mix units inside the same column without labeling every row.
Add only the context you know. Meal timing, food or carbs, medication or insulin already taken, activity and a short note can make the record easier to reconstruct later. The log records events; it does not interpret them.
- date and local time
- reading copied from your own source
- unit: mg/dL or mmol/L
- meal context plus food or carbs, when relevant
- medication or insulin record, activity and notes
Free printable blood sugar and food log
Use the printable PDF for handwriting or the CSV file for a spreadsheet. Both templates use the same neutral fields, so a paper record can be moved into a spreadsheet later without changing the structure.
Use food, meal timing, activity and notes as daily context when you have it. This preserves the useful day-by-day diary detail while keeping the sheet readable as a structured glucose logbook.
These blank templates are for record keeping only. They do not provide target ranges, calculate A1C or insulin, diagnose a condition, recommend food or medication, or replace advice from a qualified clinician.
Prepare a factual record for a doctor visit
Before an appointment, check that each reading has a date, time and unit. Add missing context only from your own notes; do not guess. Then choose the period you want to bring and export the Glytrio history as PDF or CSV.
The export is a factual diary for discussion. It does not create a diagnosis, target range, treatment plan, dose recommendation or A1C estimate.
- confirm the date, time and unit on each reading
- review meal, medication, insulin, activity and note context
- choose the relevant period in Glytrio
- export PDF for reading or CSV for structured records
- share only through a destination you choose
Offline app records and user-controlled sharing
Glytrio does not require an account for the daily diary. Records live in a local SQLite database on the device. There is no mandatory cloud copy of the diary.
PDF, CSV and backup files are created locally. Data leaves the device only when the user chooses an export destination or uses a separately described online feature. See the Privacy Policy for the exact boundaries.
